Job Title: Social Worker

Summary – The social worker will professionally apply theories and experience to navigate through complex social problems of lower socio-economic families in order to facilitate children’s medical treatment, documentation and follow-up. The social worker will address SDH (Social Determinants of Health) for each child to ensure proper care is given to the beneficiaries. The main responsibility of the social worker will be to coordinate aspects of treatment like drug purchase and follow-up.

Essential duties and responsibilities (may not include all duties of the job)

• Planning, managing and providing social services

• Coordinating patient care and treatments

• Coordinating purchase of drugs/medicines and ensuring transparency with the beneficiaries

• Administering and supervising social service programs

• Managing documents of the beneficiaries

• Documenting progress

• Participating in team meetings and keeping minutes

• Complying with Tiara’s policies

• Understanding and agreeing to support Tiara’s Vision and Mission
