Background

Doctors Without Borders/ Medecins Sans Frontieres (MSF) is an international, independent, medical humanitarian organisation that delivers emergency aid to people affected by armed conflict, epidemics, natural and man-made disasters and exclusion from healthcare in more than 70 countries. DWB offers assistance to people in need and irrespective of race, religion, gender or political affiliation. Doctors Without Borders has worked in India since 1999, providing free-of-charge essential healthcare to people in remote areas, treatment and care for people affected by HIV/AIDS, malnutrition, hepatitis C, tuberculosis, kala azar and sexual and gender-based violence. An important subject for our action is containing the effect of antibiotic resistance. We also respond to natural disasters and other emergencies, provide mental healthcare, and advocate for the development of more effective and affordable medicines to improve access to treatment everywhere. Doctors Without Borders was awarded the Indira Gandhi Prize for Peace, Disarmament and Development in 1996 and the Nobel Peace Prize in 1999. Doctor without Borders India (DWBI) runs projects in the states of Bihar, Chhattisgarh, Delhi, Jammu and Kashmir, Maharashtra, and Manipur.

Doctors Without Borders India’s (DWBI) Drug-Resistant Tuberculosis (DRTB) project in Mumbai

Free treatment, care and support to DR-TB patients

  • The collaborative project within NTEP [National Tuberculosis Elimination Program] for DRTB – patient management at Shatabdi Hospital, Govandi – OPD, community outreach medical and patient support activities in M/East Ward area
  • Medical, “Psychosocial Support” and “Infection Prevention and Control” in GTB Hospital, Sewri
  • The independent clinic in Govandi to provide free and quality care to the most severe cases of DRTB

Main Tasks & Responsibilities

  • Providing outpatient and/or inpatient paediatric care to patients/beneficiaries according to adapted and updated medical knowledge, DWBI protocols, values and universal hygiene standards to improve the new-born and child/beneficiaries’ health conditions
  • Paediatrician is going to be part of the multidisciplinary team to provide medical care for DRTB pediatric and adolescent patients in all the sites of DWBI intervention, the special focus required on small children DRTB patients.
  • Note: New-borns care is not part of the DRTB intervention in the Mumbai project.
  • Applying medical knowledge and skills to prevention, diagnosis and treatment of children, ensuring integrated newborn and paediatric care through Emergency, OPD, and IPD services in Newborns and cases of malnutrition. Prescribing the necessary treatment, and respecting DWBI protocols.
  • Collaborating closely with the different members of the team, Knowing and ensuring all updated DWBI medical protocols and universal precautions are followed and implemented, reducing bio-hazard risks and improving infection control.
  • As an expert in paediatrics, proposing strategy (modification on current newborn and paediatric care management) and implementing them after validation of Operation and Paediatric Advisor.
  • Ensuring ongoing newborn and paediatric training (formal and bedside) of the medical/paramedical multidisciplinary team in order to optimize the newborn and paediatric quality of care. Reviewing and discussing selected complicated cases or mistakes and giving technical input.
  • Keeping patients and their families informed of the illness and the treatment to follow, checking that they have been understood. Ensuring professional autonomy and confidentiality are respected and a good attitude and proper bedside manner towards the patient and their caretakers and the rest of the medical team.
  • Following up the evolution of the hospitalized/IDP patients through daily visits, consultations and examinations, prescribing the necessary treatment following DWBI protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their families about the patient’s evolution.
  • Monitoring the proper functioning of the department, equipment or material and addressing the list of the missing item for newborn and paediatric activities. Providing inputs about the general organization of the different paediatric wards, including the admission, transfer and discharge criteria + referral procedure from Maternity and post-delivery wards
  • Checking and controlling the rational distribution of medicines and equipment under his/her responsibility and taking care of the quality, disinfection and sterilization of the medical material.
  • Assuring general compliance with standing hygiene standards.
  • Participating in the introduction/improvement of the collection and analysis of epidemiological newborn and paediatric data, checking its validity and informing the line manager or project coordinator about any problems or complications of the patient’s illness and medical errors.
  • Collecting Clinical cases for further trainings and listing a series of comments to improve the current DWBI protocols
  • Managing the team under his/her direct responsibility according to DWBI HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, and directly participating in emergencies and on calls if necessary.
  • Designing and providing training to staff as required.
  • Debriefing with coordination, Headquarter (including Paediatric Advisor) and providing a written report on main findings, activities, conclusions and further recommendations.
  • Paediatrician is going to be part of the multidisciplinary team to provide medical care for DRTB pediatric and adolescent patients in all the sites of DWBI intervention, the special focus required on small children DRTB patients
  • As an expert in paediatrics, lead and adjust the current medical strategy to further improve preventive/diagnostic and treatment tools including contact tracing for better outcomes among the paediatric DRTB cohort and implementing them in all the sites of intervention after validation of Operation and Paediatric Advisor. especial emphasis to improve case finding and the diagnosis of DRTB in small children.
  • Provide medical consultation for paediatric patients, special emphasis on small children.
  • Be part of the DR TB nodal centre Committees at MEW and DWBI Clinic and ensure the quality of clinical decision making, develop tools for documentation of DRTB pediatric patients to facilitate case discussion and decision of treatment regimen by the nodal centre committee.   • Define the IPD referral system for critical paediatric patients together with MTR.

Requirements

Qualifications

Essential: MD/DNB/Diploma in paediatrics with full & valid registration from Medical Council of India or state medical councils

Experience

  • Paediatrician with 3 years of experience working as a paediatrician in the hospital.
  • Desirable working experience with DWBI or other NGOs.
  • Desirable working experience in developing countries.

  • Languages: English, Hindi & Marathi.
